Daycare/School/Camp Forms

For your safety and the safety of our staff, we ask all requested forms to be either brought in person to our office(s), emailed, or sent via portal message. You may bring them to any of our locations to begin processing. You may access our portal system on our homepage and our email is forms@vapg.com. Please ensure all sections needed by parents/guardians are complete before sending your request to avoid a delay in processing. Thank you for your cooperation.

Free: Forms will be completed for free if brought with the child to the Well-Check visit.
Time: Forms take from 7-10 business days (Mon-Fri) to complete.
Fees: $10 for a single page form; $15 for multiple page form.
Rush Fee: There will be an additional $25 fee to have a form complete within 48 business hours (Mon-Fri).

Please complete the parent portion of the form and include payment. Rush form requests must be submitted in person or have spoken with one of our front desk staff members. We will call you when the form is completed and ready for pick up. You can either pick up the form, or have the completed form emailed back to you at your requested address.

In an effort to reduce VAPG’s carbon footprint, we are going green with forms. All forms completed at the well check will be available on the patient portal by the end of the day.

If you want the form printed, there will be a charge of $10.00.
If you request a form on a day other than the well check, it will be available in 7-10 days, there will be a $15.00 fee and the form will be provided on the patient portal unless specifically requested on paper.
If you request a same day form on a day other than the well check, there will be a $25.00 additional rush fee.
