Thank you for participating in on-line medical information data gathering. CHADIS is an Internet based system from Johns Hopkins University that helps me in your child's care. The information you write must be secured for privacy and thus requires you to establish a user name and your own personal password. Additionally, I must give you an invitation code that allows you to access the system. Here is what is necessary to do:

1. Go the Internet web page: www.childhealthcare.org
   
   
2. This web page, "Center for Promotion of Child Development through Primary Care," will have a blue box on the right, and under "parents" click login
   
   
3. The next page will have another blue box where you will click "I have an invitation code"
   
   
4. The invitation code is: 7035732432
   
   
5. Fill in the boxes as instructed and submit
   
   
6. Choose the option that you have an upcoming visit in two weeks, even if you don't
   
   
7. Select check up visit option or behavior visit option and complete questionnaires and submit
